Individualised Client Support Packages

Individualised Client Support Packages, which were introduced as part of the Victorian Government’s Mental Health reforms, have replaced a number of services including Home Based Outreach, Day Programs and Care Coordination.

Individualised Client Support Packages are intended to make it easier for adults with a mental illness to get the help and support they require.  Following a comprehensive assessment a tailored support package is developed collaboratively to:

  1. develop the life skills needed to live well in the community

  2. manage mental illness

  3. find and maintain stable housing

  4. develop relationships and engage in community activities

  5. improve health and wellbeing

  6. find employment

  7. identify and connect with other relevant health and community based services.

Support packages are based around services that can be delivered in a one-on-one or group environment with an emphasis on an individual’s recovery goals.

within Australia’s primary intervention method is the Collaborative Recovery Model, developed by the University of Wollongong, Illawarra Institute for Mental Health, which focuses on achieving positive outcomes and takes a value based approach to setting life goals and managing illness experience.

ACSO Intake and Initial Assessment

Intake and screening for eligibility is undertaken by Australian Community Service Organisation (ACSO) which provides a single point for information and referral for people seeking mental health support in Gippsland.  ACSO will conduct an initial assessment in order to:

  1. determine eligibility to receive support

  2. prioritise referrals.

Following the initial assessment eligible individuals will be referred to within Australia for a comprehensive assessment.
